The Killingholme Surgery welcomes new patients.
Patients are asked to fill out a new patient registration form which can be found here.
Paper copies can be obtained from the reception.
Once you have submitted your registration documents, you may be asked to attend for a new patient health check to discuss any ongoing medical needs or medications.
The surgery covers the villages of North and South Killingholme, East Halton, Ulceby, Thornton Curtis and Habrough, Wootton, Stallingborough and Immingham.
We do welcome patients that live outside of the catchment area for out of area registration however we will not be able to provide home visits.
